Assessment of Quadratic Equation Material

Introduction

After studying quadratic equations, it is time to do an assessment to measure competency achievement. This quiz was originally prepared for students of SMPN Purworejo, Padang Ratu District, Central Lampung.

Other readers are also welcome to try the quiz or share it with their students.

Instructions

1. The quiz contains 10 questions with Multiple Choice, Checkbox, and True/False formats.
2. The time limit is 30 minutes.
3. The minimum passing criterion is 60.
4. Write your first name, last name, and email in the provided fields before starting.
5. If you only have one name, fill the last-name field with your class name. Example: first name Andi, last name 9B.
6. If you have both first and last names, add the class name after the last name. Example: Aslam Permana 9B.
7. If you do not have an email address, use a simple format such as andi@gmail.com.
8. Click Start to begin.
9. Answer each question by choosing the correct option.
10. For Checkbox questions, choose 2 or 3 answers as required.
11. After answering all questions, click Submit my answer.
12. After completion, a certificate and score report will appear.
13. The certificate can be downloaded, and the score can be saved with a screenshot as proof of completion.
